Facilities & Event Request Form

This form is intended for use ONLY for the following event type or requesting group:

All other requests should be made through the appropriate office.


Facilities and events requests should be submitted to University Scheduling no fewer than 7 days before your event.* Email events@calpoly.edu or use the form provided below. Please include all of the following information:

  1. Requestor's name, phone, and email address (If requestor is a student, be sure to include an advisor or sponsor's name and phone.)
  2. Event title
  3. Event date or dates
  4. Event start and end time
  5. Set-up and take-down times (added to the event begin and end time)
  6. Expected headcount
  7. Preferred location (if known)

*Requests for the following locations require 2 to 4 weeks (6 weeks for large events) for approval and staffing arrangements: UU 220, Chumash Auditorium, Performing Arts Center Pavilion, and Performing Arts Center Lobbies. Requests submitted fewer than 7 days prior to the event date will receive attention and facility assignment as time permits. Late requests will not receive special consideration. Confirmation that your event has been scheduled and approved will be emailed to the requestor at the address submitted on this form.
